Creating a Dialog Model

I am creating dialog models in the user interaction section of a dialog workflow component. I have multiple responses check. The form has been created as an approve/deny. When I click on the link in my process view page the window opens small and scrolled down to the bottom right. Is there a way to fix this. I have searched everywhere. Here is a screen shot of what I am talking about.
